We do not charge our models to participate. There are some designers who charge a small garment fee for the models to walk for them. If you want us to share your information with these designers please select "Yes". If you don't agree, please select "no". The can range from $25 and up.

ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM *

I am the guardian of the Model and I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault. I certify that I am physically fit, have sufficiently prepared or trained for participation in this activity, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity. I understand while participating in this activity, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns. The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREEWILL.

Non-Disparagement Clause *

The participant and legal guardian agrees that he/she/ it/ they shall not disparage, criticize or defame the production of their affiliates and prospective affiliates, designers, staff, executives, producers, models, volunteers, employees, promotions, services either publicly or privately.

Terms and Conditions *

I am the legal guardian and I agree for good and valuable consideration, the receipt of which is hereby acknowledged, hereby irrevocably authorize __Juan Carlos Guevara of Peekapose Productions __to use photographs and videos of me my child and or my property and authorize him/(her/their and his/her/their assignees), licensees, legal representatives and transferees to use and publish (with or without my name, company name, or with a fictitious name) photographs, pictures, portraits or images herein described in any and all forms and media and in all manners including composite images or distorted representations, and the purposes of publicity, illustration, commercial art, advertising, publishing (including publishing in electronic form on CDs or internet websites), for any product or services, or other lawful uses as may be determined by the photographer or Peekapose Productions. I further waive any and all rights to review or approve any uses of the images, any written copy or finished product. I am of full legal age and have read and fully understand the terms of this release. By Signing the Terms and Agreement you acknowledge this document.
